That is an interesting question. I don't have any reason to believe that it is impossible. Have you attempted to build this custom item? If so, I'd love to see the html code and help where I can.
All I have right now is the multiple options checkbox. I'm not sure if I would use a responses.xls file or write a separate script for the follow up questions about each condition?
