DirectRT skipping over trials

I'm new to DirectRT so I apologize if my error is obvious. I recently created a task similar to the go no-go task in DirectRT. The task consists of a practice block of 20 randomized trials followed by 4 blocks of 40 randomized trials. When I run the program it goes through the 20 practice trials but then when it gets to the first block of actual trials it never presents all 40 trials. It does however, present all 40 trials in the remaining blocks. I looked at the data file and it seems to be skipping over different trials every time.

Also, on occasion the trials are presented out of order. In other words, when I sort by 'order' in the data file there will occasionally be trials from blocks 2 and 3 presented interspersed with each other.

I've attached the input file and three data files that show the two problems. Any help would be greatly appreciated.

One more question. Could you please tell me exactly which trials in the input make up which blocks, especially the "first block" you referred to as the one that gets skipped?
